Registration and Participant Survey

To assist me in directing a workshop that will be as interesting and beneficial to you as possible, please briefly fill out the questionnaire below.

There are twelve fields to fill out and one for questions or comments.

When you are done, click on "Submit Form" at the bottom of the page. The form's contents will then be emailed to me.

The information is not stored on line; do not leave the page until you hit "Submit." If you want to change or add anything after submitting, you'll need to fill it out the relevant field(s) again. This is fine with me. Submit as many times - and as piecemeal - as you like.

If you have any questions about this questionnaire or about the workshop in general, please email me by clicking here. I will answer as soon as I can - most likely within 12 hours.

I would ask that you fill out as much of this questionnaire as you can, but the only required fields are the ones for your name and email address.

The questionnaire requires an email address so I can send you advance material for the workshop, as well as any notices that may be necessary. If this poses any kind of problem, please let me know by emailing me.

  10. At the beginning of the Design Process Workshop you will write a design thesis. This is an outline of an idea for a design project that you will focus on during the week. (If you have not read the description of the workshop, go here.) You will use this to explore the path a design takes from the world of imagination to the world of objects. What design project do you suppose you would like to use for this exploration?

    Choose something that is genuinely interesting to you, and relevant to your current studies or concerns. Be as specific or as general as you would like, but since the workshop takes you from the inspirational stage to the production stage in four days, a more specific thought will eventually be better. Right now, I only need to see an indication of the direction you will go in. We can, if you like, communicate via email over the next couple of weeks to focus your thoughts more clearly. Let me know if you would like my input, and I will welcome the conversation.
